This is Phase 1 of Cross Chapel’s application for the Mission Trip in June to Hokkaido. 

Please read carefully, and fill out one form per person.

This year we are prioritizing applications of Cross Chapel members.

Mission Trip Involvement

In order to maximize and best utilize your time in the scheduling of various ministry activities, we need to know how long (and when specifically) you plan to serve with us and in what capacities.

What dates are you planning to join for the mission trip?
The entire mission trip (June 4-17)
Other:

A Team — Joining for the whole mission trip (full range of responsibilities)

B Team — Joining for more than 10 days (appropriate responsibilities)

C Team — Joining for 5-10 days (less intensive responsibilities)

Visitor — Joining for less than 5 days (can join for larger events)

Partners — Committed prayer and/or financial partners
